DATA SUBJECT REQUEST FORM

You can submit your requests within the scope of your rights listed in article 11 of Personal Data Protection Law ("Law") No. 6698 to Panasonic Electric Works Elektrik Sanayi ve Ticaret Anonim Şirketi ("Company"), which acts as the data controller by one of the methods described below, according to the article 13 of the Law and article 5 of the Communiqué on the Principles and Procedures for the Request to the Data Controller ("Request Communiqué"). This request form has been prepared only for the purpose of responding to your request more easily and quickly. Other requests made in accordance with the procedures and principles organized in the Request Communiqué will also be evaluated and responded by the Company in accordance with the relevant legislation.

Your requests will be replied in writing or electronically to the contact address that you specify below, in accordance with article 6 of the Request Communiqué. Therefore, make sure that the personal data given below is accurate and that you have the authority to share this personal data.

The Company has an obligation to determine whether the data owner making the request is the real owner of the right. Within this scope, the Company has the right to request all kinds of confirmation information and documents in order to carry out identification. If a copy of your identity card is sent to the Company in order to make a confirmation of your identity, ensure that the sensitive personal data (blood type and religion information) on the relevant document is not included.

Ensure that the information and documents submitted to the Company within the scope of your request do not include any sensitive personal data (e.g. health information, religion information, political opinion information) that is considered in a limited number in article 6 of the Law.

We will finalize your request without any charge as soon as possible and in any case within 30 (thirty) days at the latest, depending on its nature, from the date it is obtained by the Company. If there is an additional cost for the finalization of your

request, the fees in the tariff that is determined by the Personal Data Protection Board may be requested by you.

In line with the information, documents and requests that I have provided in this request form, I request that my application to your Company be evaluated in accordance with the provisions of the Law and the Request Communiqué and I request to be informed. Within this scope, I declare and undertake that I have been informed that the information and documents I have provided to you with this application herein are accurate and up-to-date, that your Company may process my personal data, request additional information and/or documents in order to finalize my application, and I may be required to pay the fee determined by the Personal Data Protection Board if it requires an extra cost.
